• Current within the last 3 years. However, discretion will be used in accepting documentation of conditions which are permanent or non-varying. Some conditions that are chronic and/or changing may require more current documentation and/or frequent updates.


• Documentation must include both diagnostic information and an explanation, on letterhead of certified professional, of the current manifestations or functional limitations on the condition. It should be thorough enough to demonstrate whether or not a major life activity is substantially limited (including the extent, duration, impact of the condition, and impact of medication).


• Documentation must be thorough enough to support the accommodations that are being considered.
