Purpose of Documentation    

Ø       To verify the existence of a disability

Ø       To support the accommodation being considered

Ø       To personalize the student’s right to equal access to the institution

   
Pending Documentation    

DSSO reserves the right to deny services and/or accommodations.  At DSSO’s discretion, limited services can be provided for one semester while documentation is being submitted to department. 

   
Incomplete or Inadequate Documentation    

If the initial documentation is incomplete or inadequate DSSO has the discretion to require additional information.  Any cost-related expenses for obtaining documentation are borne by the student.

   
Documentation, Whose Responsibility?    

It is the responsibility of the student seeking services to provide DSSO documentation related to a disability from the appropriate licensed professional to verify the existence of a disability and need for an accommodation. 
   
Documentation Needs To Be    

Current within the last 3 years . 

However, discretion will be used in accepting documentation of conditions, which are permanent or non-varying.  Some conditions that are chronic and/or changing may require more current documentation and/or frequent updates.  A chronic or progressive condition must include the degree and range of functioning.

 

   
In General, Documentation Should Include    

Ø       Identification of the nature and extent of the disability

Ø       Specific information on the functional imitations as related to the academic environment

Ø       Description of the current course of treatment including medical side effects
