• A List of Emergency Phone Numbers
• Your Doctor's Phone Number
• Your Hospital Preference
• The $15.00 Registration Fee

Please plan an hour of your time for paperwork and orientation

Requests for care are taken the preceding semester:
• Submit request for fall semester by July 1
• Submit request for spring semester by Nov 1
• Submit request for spring semester by April 1
