Here you can download both the team & the individual entry form. Once you fill in the information you can drop the form off at memorial gym room 103 from 8:00 am to 10:00 pm or you can send it as an attachment to recsports@utep.edu
*Note. All forms must be signed and sent before the entry deadline date is passed, forms receive after that will not be admitted unless you have the prior approval of the Recreational Sports Department.
